APPOINTMENTS
Office hours are by appointment only; however, the practice accommodates today's busy lifestyle. In addition to traditional business hours, convenient weekday evening appointments are offered, as well as on Saturdays. INITIAL VISIT INFORMATION
For your first appointment, please bring valid ID as well as your preferred method of payment. If you plan to use insurance, bring your insurance identification card. Please download the Client Information form, Psychological Services Agreement, HIPAA Notice, and any other applicable documentation prior to your Initial Appointment by going to the SERVICES page. Please review the forms at your convenience, and bring them completed to your appointment. You may also submit the completed Client Information form and the Agreement signature page in advance via email or FAX, to save time and help preserve the environment.
EMERGENCIES
Dr. Pizzi is not available for emergencies. In case of a medical or psychiatric emergency and you cannot reach the doctor, call your local emergency phone number (i.e., 911) or proceed to the nearest hospital emergency room and ask for the psychologist or psychiatrist on call.
